Live well at home (following an assessment of need)

These teams help to find or put in place bespoke care packages for people who have an assessed care and support need. The level of involvement from Council services will depend on:

  • a person's needs
  • who is funding the care package
  • whether we are setting up a service on someone else's behalf

This will be different for each person and we will explain processes to help you understand. It is important that you understand this so make sure you ask any questions you have.

Bolton council services that could support you include:

Brokerage
This team help to set up services like home care for you. This includes sourcing services, making sure services are set up - they will make arrangements like what services are delivered, where they will be delivered, times and costs.

Home support reablement
This service offers people support in their on homes for a set number of weeks to help then carry out day to day activities while their recuperate.  

Intermediate care
This service offers short term bed-based care for people who:

  • need time to recuperate before going home following an incident eg a fall or a stroke, or
  • have been discharged from hospital because there is no medical reason for them to be there, but who need to be assessed to ensure it’s safe for them to go home.
