In Bolton we have adopted the adult social care vision that social care future (a people-powered movement for change) have codesigned with people accessing adult social care services.

We all want to live in the place we call home, with the people and things we love, in communities where we look out for one another, doing things that matter to us.

To help us deliver this vision, in Bolton we have set three priorities:

Living well at home

Connecting and developing services that help Bolton residents to live safely in their own home and community. This includes:

  • a range of information and advice
  • equipment and adaptations to help day to day living
  • services that go into communities or people's homes to socialise or undertake specific tasks like shopping or supporting bedtime routines
  • support for informal carers – families and friends who help someone with a care and support need

Making the best use of our resources

Focusing funding and staff teams on the tasks and services that deliver our statutory duties in the best possible way to make a real difference for Bolton residents.

Working together in partnership

Working together across Bolton and regionally across Greater Manchester and the North West to have co-ordinated plans and to learn from each other so people can live independently and have choice and control over their own life for as long as possible, and to have a co-ordinated approach to health and care planning when needed. Bolton's locality plan 2024-2029 is our shared plan for health and care in Bolton.
