What is living well at home

Living well at home is our local priority for adult social care to help you live in the place and community that you call home, with the people and things you love.  

Many people think of adult social care as residential and nursing care homes, but these are only one part of the adult social care offer in Bolton and are services that not everyone will need.  

Living well at home for many people will be what you see as your day-to-day life: 

  • having a healthy lifestyle 
  • eating healthy 
  • having somewhere to live 
  • earning money, or accessing any benefits that you are entitled to 
  • managing your money 
  • keeping active 
  • connecting with others  

Some people may not need any support to have these things in place, but others may want some trusted information and advice to help manage these.    

Whilst you may not currently have a need for adult social care services, we want to help you live independently with choice and control for as long as possible, proving it is safe for you to do so. Our living well at home offer also includes: 

  • information and advice 
  • equipment and adaptations for your home (with or without an assessment of need) 
  • support groups 
  • day activities  
  • services that support you in your home to help with activities like getting in and out of bed, preparing meals and personal care for short or longer periods of time 
  • specialist housing schemes where there are people on hand to support you 

Where it is not possible for you to live safely in a home you may have lived in for some time, residential or nursing care may become your home. The services that they offer will help you to live well at home by helping you to carry out your day to day activities and connecting with other people.   

Living well at home may look different for different people, because it is personalised to you, or the person you care for. Some services that you receive to help you live well at home may need you to have an assessment of need first and a financial assessment to determine whether you can access any financial support.
