What is an assessment or a review?

A care act assessment

This is a process that identifies a person's needs for care and support and how these needs impact on their wellbeing. The assessment will identify a person's strengths and will help understand any care and support. Care act needs cover physical, emotional and social needs. Friends and relatives who offer a person support on day to-day basis are known as a carer and may also be part of this conversation. We will try to carry out an assessment in the person's home but we can adapt this if this does not meet your needs.

A carer assessment

This is a conversation between a person who offers day to day support for someone who has care and support needs (a carer) and the local authority. This is to understand what help and support the carer may need to keep them fit and well and to be able to continue in their caring role. We will try to carry out an assessment in the carer's home, but we can adapt this.

Care needs review and carers review

These happen approximately every 12 months after an assessment (or sooner if something changes). They are a conversation to see how things are going and check that plans continue to support you.

Care needs assessments, carers assessments and reviews are all free of charge.
