What happens once I have asked for a care needs or carers assessment?

Once your details have been collected, they will be shared with one of our adult social care neighbourhood teams who work across Bolton. They will arrange a suitable time and place for your assessment and make arrangements if you have any communication needs e.g. a translator.

If you have assessed needs and a care plan is put into place, the relevant neighbourhood team team will then become your point of contact. If your plan will be in place for a longer period of time, the neighbourhood  team will try to meet with you for a review every 12 months, or sooner if needed.
