How do I get a care act assessment?
There are a different ways that you can get a needs assessment depending on your circumstances:
Early intervention team
You can contact our early intervention team to request an assessment. They will take some details from you to start the process.
Neighbourhood adult social care teams
There are 6 neighbourhood adult social care teams that work across Bolton and fulfil our statutory responsibilities in assessing your care act needs. The early intervention team will be able to correctly signpost you to your appropriate neighbourhood team if required.
Community learning and disability (CLD) and transition team
The CLD team assess and support the health and care needs of Bolton adults who have a learning disability. The transition team support young adults with ongoing care and support needs to move from children's social care to adult social care with the team being involved in assessments or reviews to support this process.
Integrated discharge team
If you have been admitted to hospital, this team may undertake an assessment of need before your discharge if this is required.
The admission avoidance team
Provides urgent assessment and intervention to Bolton residents over the age of 18 if they are at risk of admission to hospital. The aim is to meet people's needs in the community and reduce avoidable admissions to hospital. This is a multiagency team that includes social workers, mental health workers, nurses and pharmacists for example.
