It is important to us that you receive the right information at the right time to support:

- you to help plan the care needs for someone that you care for
- your needs as a carer
You may be looking for information and advice to:
- think about longer-term care needs (planning for the future)
- manage an emerging situation to help you live well at home (becoming less steady when walking, for example)
- understand the services that are available to you when something has happened, and it is difficult for you to live well at home (you are no longer able to get out of bed by yourself, for example)
Some information that you need will be about services and processes that are delivered nationally or regionally. Some will be about services and processes delivered in Bolton.
