Assessment and review
These services are in place to have conversations with you to help put a plan in place for your care and support needs (care assessment), or the support you need as a carer (carers assessment).

Early intervention team
You can contact our early intervention team to request an assessment. They will take some details from you to start the process. While you wait for an assessment, our early intervention team will be your point of contact. They will be able to continue giving you information and advice. You can also refer to our website and our directory of services.
Our assessment and care management district teams
Once your details have been collected, they will be shared with one of our district teams who work in neighbourhoods across Bolton. They will arrange a suitable time and place for your assessment, and make arrangements for any support you may need eg a translator.
If you have assessed needs and a care plan is put into place, the district team will become your point of contact. If your plan will be in place for a longer period of time, the district team will meet with you for a review every 12 months, or sooner if needed. Community learning and disability (CLD) team and transitions team
The CLD team assess and support the health and care needs of Bolton adults who have a learning disability. The transition team support young adults with ongoing care and support needs to move from children's social care to adult social care with the team being involved in assessments or reviews to support this process.
Hospital discharge team
If you have been admitted to hospital, this team may undertake an assessment of need before you are medically discharged. Whilst the hospital will discharge you because you are medically fit, you may be assessed to make sure your care and support needs are met.
Admission avoidance team
The team provides urgent assessment and intervention to Bolton residents over the age of 18 if they are at risk of admission to hospital. The aim is to meet people's needs in the community and reduce avoidable admissions to hospital. This is a multiagency team that includes social workers, mental health workers, nurses and pharmacists for example.
Finance, income and assessment team (FIAT)
After an assessment of care and support needs you will be offered a financial assessment to help understand what financial support may be available to support your care needs.
