What do I need to provide?
- one recent passport sized photograph
(please make sure your photograph conforms to standard passport photo rules) - a copy of a document proving your identity
- a copy of a document confirming your address
- proof of any benefit that means you qualify automatically (dated in the last 12 months)
- a payment of £10
Your documents should either be:
- emailed to: csdbluebadges@bolton.gov.uk (Please include name, and tracking reference number)
- posted to: The Blue Badge Team, One Stop Shop, Bolton Town Hall, Victoria Square, BL1 1RU (Please include name and address)
Applications will not be assessed until photographs and proofs have been received.
