In December 2024 we outlined important changes to the medical standard and documentation.
The transition to the new medical standard and documentation commence today - 22nd of January 2025. The changes are as follows:
1. The current Bolton Council medical documentation (Full Certificate of Medical Fitness and Annual Statement of Medical Fitness) will be replaced by the 'DVLA Medical Examination Report' (also known as the D4) and a new Bolton Council Medical Declaration.

3. You can now also choose who completes the D4 and Bolton Council Medical Declaration. This can be your normal GP based in the UK, or any General Medical Council (GMC) registered GP who lives and works in the UK, both must be licensed to practice by the GMC. Not all GP's will carry out the eye test, which is required as part of the D4, so you may also need an appointment with an Optician.
Medical documentation completed in the old format (Full Certificate of Medical Fitness and Annual Statement of Medical Fitness) can still be submitted during the transition period.
However, from the 22nd May 2025 this documentation will no longer be valid.
You are reminded that the conditions attached to your licence require you to notify the Council of any newly diagnosed or change to a current medical condition which may restrict your entitlement to a driver's licence requiring a DVSA Group 2 medical standard.
Notification must be sent to the Council's email address immediately (or in any case within 48 hours) of the relevant diagnosis or change to medical condition.
