Share your thoughts on bringing together services
Bolton Integrated Care Partnership are seeking the views of Bolton people on plans to transform health and social care services by bringing together community health services, adult social care and aspects of primary care as part of an Integrated Care Partnership.
The partnership brings together Bolton Council, Bolton NHS Foundation Trust (FT) and Bolton Clinical Commissioning Group (CCG) working closesly with GP services through our Primary Care Networks,Greater Manchester Mental Health Trust, the voluntary sector and wider services including housing and police.
The focus of the ICP is to work together as one team, offering co-ordinated care which is closer to people’s homes, in a neighbourhood-based model of care, tailored to meet individual and community needs. The aim is to ensure that service users, carers and their families experience better services when they need them, where people can access services more simply and don’t have to tell multiple health or care professionals the same information.
The partnership will also focus more heavily on working with pepole to help them prevent illness or problems themselves and to make make better use of people’s own strengths and abiltities to help themselves which can often be overlooked.
Changes are already happening to ensure that our services are the best they can be and that care is better coordinated around people’s needs. However, we want to make it easier for our teams to work together by becoming a more formalised Alliance arrangement. The Integrated Care Partnership Alliance will give us the official arrangements we need for staff from different organisations to work more closely together to provide their care close to home when people need it.
